1. Accepting these terms
By accessing or using Insimen Medical Record, you agree to these Terms and the applicable Privacy Policy. If you use the product for an organization, you confirm that you are authorized to agree on its behalf.
2. Accounts
Access uses an Insimen account used with the relevant facility authorization and role; clinical access is not granted merely because someone is signed in. You are responsible for maintaining accurate account information and safeguarding your credentials.
3. Acceptable use
Use the service lawfully and for its intended purpose.

4. Your responsibilities
You remain responsible for how you use the service, the data you enter, the instructions you give, and decisions you make based on product information.
- facilities determine the processing basis, staff authority, retention, and clinical-data quality required by applicable obligations
- healthcare professionals use their access only for lawful care needs
- patients keep account credentials secure and provide accurate information during appointment or screening processes
5. Product-specific terms
The following points apply specifically to this product.
- Facilities remain responsible for user authority, care-record accuracy, retention, and compliance with applicable health regulations.
- Deleting or changing clinical records follows authorization, history trails, and established correction procedures to preserve safety and auditability.
6. Roles of facilities, healthcare professionals, and patients
Medical Record is an operational support system for healthcare facilities. A facility remains responsible for deciding who is authorized, how care is delivered, data quality, retention, and compliance with applicable health obligations. Healthcare professionals may use access only for lawful duties and care needs.
Patients may use features made available to them, such as appointments, queues, service information, or access to certain records. Being signed in does not automatically grant clinical access, and patient access does not mean the patient may overwrite a facility or professional record.
- Do not share accounts, access cards, passwords, one-time codes, or sessions with staff, patients, or others.
- Facilities must disable staff access when a role changes, employment ends, or data is no longer needed.
- Everyone must use patient data only for a lawful and relevant purpose.
7. Care, appointments, queues, and clinical records
Appointments, queues, care notes, prescriptions, pharmacy records, and other records must be created or changed by authorized people. The system supports coordination but does not replace an examination, clinical decision, informed consent, or a facility's communication with a patient.
Use the available correction procedure when an error occurs. Clinical records must not be casually deleted or overwritten because a change history may be necessary for patient safety and audit.
- Check patient identity and facility context before creating a record or prescription.
- Do not use patient data for demos, exports, or communications outside a lawful purpose.
- Escalate a safety incident, wrong-patient event, or prescription error through the facility's internal path.
8. Documents, prescriptions, pharmacy, and administration
Care documents, prescriptions, pharmacy data, and other attachments may be highly sensitive. Upload, download, change, and deletion features follow role, facility, access status, and audit processes. Data must not be moved to a public folder or shared through an open link.
Administrative actions such as changing roles, managing a facility, exporting data, or correcting a record may require additional verification and be recorded as significant activity.
- Use facility-managed devices and networks when required by internal policy.
- Do not send clinical-record contents through a general channel not designed for them.
- Confirm the recipient, facility, and purpose before downloading or sending a document.
9. Availability, integrations, and system limits
Medical Record may depend on connectivity, devices, notification services, hosting providers, or facility integrations. A system outage may delay information, so facilities should maintain suitable continuity and manual procedures.
Insimen provides supporting software, not clinical decisions, accreditation, practice licensing, medical-outcome guarantees, or emergency services. For emergencies, use the facility's emergency path and local services.
10. Changes, suspension, and access termination
Access may be suspended to protect patients, investigate abuse, meet legal requirements, or respond to an authority change. Closing an account or facility does not automatically erase care records that must be retained; transfer, archive, and retention must follow the agreed procedure.
We may update modules and security controls with appropriate notice. A facility must still assess the effect on its procedures and staff access.
11. Fees and third-party services
If a service is paid, fees, scope, schedule, and payment method are presented before the relevant order or confirmation. Third-party services may have their own terms; we do not assume their obligations unless expressly stated.
12. Suspension and termination
Access may be limited when necessary for security, legal compliance, abuse investigation, or user protection. Where safe and possible, we will explain or provide a suitable recovery path.
13. Changes and contact
We may update these terms when the product, law, or security practice changes. The update date appears on this page. Continued use after an effective change means you accept the updated terms to the extent permitted by law.
Medical Record is an operational and documentation support system. It does not replace clinical assessment, healthcare-professional judgment, professional standards, or a healthcare facility's legal obligations.
